Open Component Separation for Ventral Hernias

Abstract
Incisional ventral hernias are common complications after laparotomy. Abdominal wall reconstruction of a very large ventral wall defect is a significant challenge to both surgeons and patients. Component separation of the abdominal musculature is a useful technique for these conditions, by releasing one or more myofascial planes to close large ventral defects. It can be performed by either an open or a laparoscopic approach. The present review describes a brief history, surgical anatomy, perioperative management, surgical procedure, postoperative complications, and results of open component separation for ventral hernias. Because of the lack of well-designed randomized controlled studies evaluating the outcomes of various techniques of open component separation for ventral hernias, definite conclusions about optimal treatments for large ventral hernias still remains to be clarified.
